NOTICE OF INTENT
MITIGATED NEGATIVE DECLARATION AND PUBLIC HEARING


LEAD AGENCY:        City of Lakeport

PROJECT TITLE:        AMG and Associates
            36 Unit Senior Affordable Development

PROJECT LOCATION:    1255 Martin Street, Lakeport, California     APN:  025-431-16

PROJECT DESCRIPTION:  California Environmental Quality Act (CEQA) Initial Study (IS) for the proposed development of 36 attached residential units for income-qualified seniors.  The project includes five two-story structures - under a unified roof, a community center, pool, on-site parking, landscaping, and storm water retention area. 

The project site is not listed on the Hazardous Waste and Substances Sites List as set forth in Government Code Section 65962.5.

FINDINGS/DETERMINATION:  Lakeport City staff has reviewed the proposed project, prepared a CEQA Initial Study and determined that the project will not have a significant effect on the environment, provided mitigation measure are implemented.  The City proposes adoption of a Mitigated Negative Declaration (MND) for this project.

PUBLIC REVIEW PERIOD:  The public review period for the IS/MND will commence on June 1, 2015, and end on June 24, 2015. Interested and concerned individuals, and public agencies may submit written comments on the CEQA IS/MND within the public review period, to the address below.  Copies of the IS/MND are available for review at Lakeport City Hall, 225 Park Street - Lakeport. 

PUBLIC HEARING / MEETING:  The Planning Commission of the City of Lakeport will hold a public hearing to consider these applications, the Initial Study, and the adoption of a Mitigated Negative Declaration of environmental impact on Wednesday, June 24, 2015, at 6:00 p.m. in the Council Chambers of City Hall, 225 Park Street in Lakeport, California. NOTICE OF PUBLIC HEARING AND MITIGATED NEGATIVE DECLARATION


NOTICE IS HEREBY GIVEN that the Planning Commission of the County of Lake, State of California, will hold a public hearing on Major Use Permit application UP 15-01, proposing  construction of a +/- 9,100 square foot retail store. 

The project applicant is Cross Development on behalf of Dollar General. 

The Planning Commission will consider adoption of a mitigated negative declaration for this project based on Initial Study IS 15-03. 

The project is located at 4315 Douglas Street and 5505 Main Street, Kelseyville, CA, and further described as APN 008-710-50 and 008-710-51.

The project is proposed in the C2-DR (Community Commercial-Design Review) zoning district, on a total of 1.32 acres of land. 

The proposed mitigated negative declaration will be available for review at the Community Development Department, Lake County Courthouse, 255 North Forbes Street, Lakeport, California, twenty (20) days before the hearing, and the staff report will be available ten (10) days before the hearing.

NOTICE IS HEREBY GIVEN that the Board of Supervisors, County of Lake, State of California, has set TUESDAY, JUNE 2, 2015, at 9:15 A.M., in the Board of Supervisors Chambers, Courthouse, 255 North Forbes Street, Lakeport, as the time and place for conducting a PUBLIC HEARING regarding a proposed Resolution approving Resolutions and Capital Fire Facility and Equipment Plans submitted by Lake County Fire Agencies and updating the Lake County Capital Fire Facility and Equipment Plan and authorizing continued collection of Fire Mitigation Fees.

Certified copies of the proposed Resolution are available at the office of the Clerk of the Board of Supervisors, 255 North Forbes Street, Room 109, Lakeport, CA 95453.
