NOTICE OF LAND USE APPLICATION PUBLIC HEARING

AND INTENT TO ADOPT A MITIGATED NEGATIVE DECLARATION

PROJECT TITLE: Lake County Tribal Health


PROJECT LOCATION: 925 Bevins Court APN: 025-462-23


PROJECT DESCRIPTION: Architectural & Design Review and Environmental Review to allow the construction of a 19,680 SF 2-story addition to an existing outpatient medical clinic, interior renovations to the existing 6,151 SF clinic, and related site improvements. Deviation of off-street parking is also requested.


The project is not listed on the Hazardous Waste and Substances Sites List as set forth in Government Code Section 65962.5.


FINDINGS / DETERMINATION: The City has reviewed and considered the proposed project and has determined that the project will not have a significant effect on the environment, with substantial supporting evidence provided in the Initial Study. The City hereby prepares and proposes to adopt a Mitigated Negative Declaration for this project.


PUBLIC REVIEW PERIOD: A 20-day public review period for the Mitigated Negative Declaration will commence on March 6, 2009, and end on March 25, 2009, for interested and concerned individuals and public agencies to submit written comments on the document. Any written comments on the Mitigated Negative Declaration must be received within the public review period. Copies of the Mitigated Negative Declaration are available for review at City Hall, 225 Park Street, Lakeport.


PUBLIC HEARING / MEETING: The Planning Commission of the City of Lakeport will hold a public hearing to consider these applications, the Initial Study, and the adoption of a Mitigated Negative Declaration of environmental impact on Wednesday, March 25, 2009, at 6:00 p.m., in the Council Chambers of City Hall, located at 225 Park Street, at which time and place all interested persons may appear and be heard. A copy of the Initial Study is available for review at Lakeport City Hall.
