Sutter Lakeside Hospital: Minor use permit extension for modular clinic

NOTICE OF PROPOSED MINOR USE PERMIT EXTENSION

LAKE COUNTY COMMUNITY DEVELOPMENT DEPARTMENT


NOTICE IS HEREBY GIVEN that the Lake County Community Development Director will consider approving a minor use permit extension (MUPX 11-01) to allow installation of a 4800-square foot modular clinic at Sutter Lakeside Hospital to remain for one additional five year period, located at 5176 Hill Road, Lakeport, APN 029-041-24.


The Community Development Director will reconsider the previously adopted mitigated negative declaration based on Initial Study IS 06-53. The previously approved negative declaration and the proposed conditions of approval are available from the Community Development Department Lake County Courthouse, 255 North Forbes Street, Lakeport CA, 95453.

NOTICE IS HEREBY FURTHER GIVEN that the Community Development Director will approve this MINOR USE PERMIT with no public hearing if no written request for a public hearing is submitted to the Planning Division of the Community Development Department by 5:00 P.M., August 4, 2011. Should a timely request for hearing be filed, a public hearing will be held on August 10, 20011 at 2PM in Conference Room “B” on the first floor the Courthouse, 255 N. Forbes St., Lakeport, California.
