If you are a current Tribal Health patient age 50 years or older and would like to be placed on the list, call 707-263-8382, Extension 9, or visit www.LCTHC.com to apply online.

When calling, leaving a message or contacting Tribal Health via the Internet, please have the following information ready:

– First and last name;
– Date of birth;
– Phone number;
– If you are a current Lake County Tribal Health patient;
– If you are American Indian or Alaska Native.
